Supporting Documents

Please bring to your appointment at least one of the following original documents:

  • Fully-valid, undamaged U.S. passport (may be expired)
  • U.S. birth certificate that meets the following requirements:
    • Issued by the city, county, or state of birth
    • Lists your full name, date of birth, and place of birth
    • Lists your parent(s)’ full names
    • Has the date filed with registrar’s office
    • Has the registrar’s signature
    • Has the seal of the issuing authority
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization
  • Certificate of Citizenship
  1. You must bring a clear photocopy of the passport applicant’s bio page, including the signature page.  For each applicant under the age of 16, you need to bring photocopies of both parents’ passports.
  2. You must also bring photocopies of any original documents required to be submitted with the passport application, i.e. birth certificate, marriage certificate, death certificate, divorce decree, adoption decree.
  3. It is important that the photocopies be clear and match the original documents you are presenting for inspection.  If the copy does not match the original, it will not be accepted and we may charge you US$1.00 for each new copy we must make.
  4. If you do not bring your own copies, we can make copies for you.  However, please note that we charge US$1.00 per copy.
